Referring a patient to Salish Cancer Center is easy!

Providers can start the referral process by simply filling out the referral information form.

Our providers ensure to keep referring providers up-to-date on their patient’s care.

Records needed for new patients:

Oncology Patients Records

  • Chart Notes (last 90 days and up to date of diagnosis up to 1 year back of records pertaining to diagnosis/specialty)
  • ED visit notes
  • Imaging reports
  • Surgical reports
  • Biopsy and pathology reports (biopsy reports = OP notes/procedure notes)
  • Mammograms (2 years back) DEXA Scans, EGD, Colonoscopy, ECHO, EKG
  • Lab Results and reports
  • Genetic counseling and testing 
  • Targeted Gene Therapy results and reports

Hematology Patient Records

  • Last year of lab results
  • Last year chart notes (PCP, Specialists, and/or ED Notes)
  • Colonoscopy & pathology reports 

Naturopathic Onc Patient Records

  • See Oncology Patient Records Needs
  • If patient has previously seen a naturopath the last chart note is needed

Acupuncture Patient Records

  • Most recent PCP OV note 
  • Any radiology reports significant to referral diagnosis in last year
